Book of Remembrance
Congregation Bet Shalom will distribute a Book of Remembrance at all Yizkor services this year. This dignified way to remember loved ones and friends will also contain Psalms, Jewish
mourning customs, and the Mourner’s Kaddish.
There will be no fee for the inclusion of names in this Book of
Remembrance. If you wish, you may make a tzedakah donation to honor
the memories of loved ones, but there is no obligation to do so.
This is the format of how the names will look which you submit below for
inclusion in the Book of Remembrance:

For inclusion in the book, please fill out this form with the names you wish to be included. You will have several choices.
1. You may keep your listing the same as it was last year. See last year's listings here.
2. You may add additional names to last year's listing.
3. If you have not listed names in the past, simply fill in the complete form.
ALL NAMES MUST BE RECEIVED BY END OF BUSINESS DAY THURSDAY, AUGUST 31 IN ORDER TO HAVE THEM INCLUDED!
